Is it very likely that if you were waged in battle, and a lot was going on around you, that you might accidentaly stab an ally?

only once before another of your allies 'accidentally' stabs you.
Seriously though, I suppose that could happen ... it would be more likely with fighters unused to fighting in groups, than trained soldiers, I'd suggest.

Of course, it depends on a lot of things, I'd imagine ...
- mist or similar might make it difficult to distinguish between friends and foes
- you might get caught up in the heat of battle and lose control
- you might really be working for the enemies
And I suppose there's always the chance that your companion looks similar in some way to one of the enemy and mistaking them or something. The evil twin syndrome.

Ha, ok I like the mist idea and the 'battle rage.' It works perfectley for my character who has battle rage acutally.

I'd say it also depends on the size of the battle. Even trained soldiers (in a medieval setting of course, I'm not talking about modern warfare) will have difficulty distinguishing between friend and foe, if the battle and confusion is large enough (I.E., the larger the battle, the worse the confusion). Just remember not to make it a full-out war, because we haven't had those in Sarvonia for quite a number of years
