Generally I prefer it that things that are on the site in pretty elaborate form (like a map that shows the provincial borders) aren't changed unless there are very important reasons for it. So the challenge then is basically to work on interpreting a map in a creative way, in which I think we're usually quite good at.

I'm up to the challenge :) But let's see if I understood the information correctly: Ravenport is part of the Duchy of Margulf. As it is the only designated town in the area, I assume it is the capital. But there's also this Rhenos Castle I noted, is this the seat of the Duke?

I have a few ideas in mind why it would have kept small anyway, I was going to make it besieged several times, more like a garrison which has trouble expanding because of it's heavy fortifications (moat and walls etc.) and afterwards have it grow more vertically then horizontally, which would be unique to Santharia as far as I know (apart from the perhaps dwarven cities which I imagine can be more vertically aligned as well). The moats I want to create into a park, much like Valencia in Spain (read about the Turia Gardens here). I get that idea from a city in Morrowind, which is also spread over several levels. The upper levels would be reserved for administration, the lower level for storage houses, commerce and poorer houses, also including a subterranean level.

Fox had a Morrowwind town in his mind as well when creating Thalambath, let's see, what different concepts will arise from the same idea! :D

Well, if I'm remembering correctly what I read in a book last year, it is usually the case, in a medieval system, that feudal lords would rule over rural areas, while urban areas, even if they are nominally part of a noble's fief, are largely self-governing.  I think in exchange for that freedom they pay taxes and contribute soldiers or something like that, can't really remember that part very well. 

So I guess what I'm saying is, it doesn't seem that odd for even a fairly large city to not have its own administrative area; cities rule themselves and nobles rule the surrounding rural areas.
